Registered pharmacist consent letter for a drug licence

A letter from a Registered Pharmacist saying he or she agrees to work as the qualified person in charge of a pharmacy or medical store, for the drug licence application (Forms 20 and 21).

This is a sample, not an official form. Offices differ. Check this text with the issuing office before you sign, and use the office's own form if it gives you one. Govt License does not give legal advice.

What it is for

Use it when you apply for a retail drug licence (pharmacy licence, medical store licence or chemist licence). The Drugs Rules, 1945 need a Registered Pharmacist in charge of a retail pharmacy, and most state offices ask for the pharmacist's consent or appointment letter with the application. Some offices give their own format, so check first.

Who signs

The Registered Pharmacist signs. The owner of the firm can countersign to approve the appointment.

Common questions

Is a pharmacist consent letter needed for a drug licence?
Most state offices ask for the pharmacist's consent or appointment letter, the registration certificate and a declaration, because a retail pharmacy needs a Registered Pharmacist in charge. Check your state portal's list.
Can one pharmacist be in charge of two shops?
A Registered Pharmacist should be in charge of the pharmacy he or she is named for, and many offices ask for a statement that the pharmacist works at no other pharmacy. Check with your office.
Do I need this for a wholesale licence?
A wholesale licence needs a competent person in charge, who may be a Registered Pharmacist or an experienced person. The office may ask for a similar consent. Check your state checklist.
