Affidavit format for trade licence
A sworn statement made before a notary, used by some municipal offices as proof that the premises and details in a trade licence application are true.
What it is for
Use it only when the licence office asks for an affidavit. Many cities accept a self-declaration instead.

How to execute this document
- Stamp paper: Offices usually ask for non-judicial stamp paper of a small value that your state sets. Check with the office you will submit to.
- Notarisation: Sign in front of a Notary Public, who attests the affidavit (Notaries Act, 1952).
- Registration: Not needed.
- Who signs: The applicant signs in front of a notary public or oath commissioner, who adds a stamp and seal.
- Witnesses: none needed unless the office asks.
- ID and photos: Carry an original ID of the person who signs. Never write an Aadhaar number on the document.
- Where to submit: The office or portal that asked for it, together with the other documents in the checklist.
Common mistakes
- Signing before you reach the notary.
- Going without original ID.
- Using stamp paper of a value the office does not accept.
- Leaving a page unsigned.
