Registered pharmacist affidavit and declaration for a drug licence

A sworn statement by a Registered Pharmacist giving registration details and confirming he or she will personally supervise the pharmacy named in the drug licence application.

This is a sample, not an official form. Offices differ. Check this text with the issuing office before you sign, and use the office's own form if it gives you one. Govt License does not give legal advice.

What it is for

Use it when the licensing office asks for an affidavit or declaration from the pharmacist along with the consent letter. It is usually printed on non-judicial stamp paper of the value your state asks for and notarised. Some offices give their own format, so check first.

Who signs

The pharmacist signs in front of a notary or oath commissioner.
