General affidavit format

A sworn written statement of facts made by one person, which can be used for many purposes where an office asks for an affidavit.

This is a sample, not an official form. Offices differ. Check this text with the issuing office before you sign, and use the office's own form if it gives you one. Govt License does not give legal advice.

What it is for

Use it when an office, bank, school or court asks for an affidavit and gives no format. Write the facts you are stating, one on each line, in simple words.

Who signs

The person making the statement (the deponent) signs in front of a notary public or oath commissioner, who attests it.
