How to apply

  1. Open swavlambancard.gov.in and register
  2. Fill the application and upload the photo, signature and ID
  3. Submit it and note the receipt number
  4. You get an SMS with the hospital and date for the assessment
  5. Visit the hospital and the medical board decides the type and percentage of disability
  6. The UDID card is generated and sent free by speed post
  7. Track the application on the same portal
Good to know
  • DEPwD says applying is free, with the card delivered free by speed post.
  • If you already have a disability certificate, you can use it while applying; the portal and your district welfare office explain how.
  • You can ask for renewal about 9 months before the card expires.
  • Beware of agents who ask money to apply.

Is there a fee for the UDID card?
No. DEPwD lists the fee as nil and the card is posted free of cost by speed post.
How do I apply for UDID?
Register and apply on swavlambancard.gov.in. After submission the application goes to the district Chief Medical Officer, who assigns a specialist for the assessment.
How do I check the UDID status?
Use Track Your Application Status on the portal with the enrolment number, UDID number, Aadhaar or registered mobile number.
