After you apply
What to do once the form is submitted: where to track it, how long it takes, why it gets rejected and what the law says if you do not have it.
If you do not have it or miss the deadline
If a person who must report a birth or death fails to do so without good reason, or gives false information, the fine can go up to ₹250. For hospital heads and other listed persons the fine can go up to ₹1,000 for each birth or death. A Registrar who does not give the certificate can also be fined up to ₹250. Late reports after 30 days need written permission and a fee, and after one year a magistrate's order.

Common reasons for rejection and how to fix them
| Reason | How to fix it |
|---|---|
| The death was reported late, after 30 days | Ask for written permission from the District Registrar and pay the fee set by your state; after one year you need an order from the District or Sub-Divisional Magistrate. |
| The name of the person who died has initials or short forms | Write the full name as first name, middle name and last name with no short forms, as the 2024 model rules ask. |
| The person giving the information did not sign the form | Sign the form as the informant, as the law now requires a signature. |
| The medical certificate of cause of death is missing | Ask the hospital or doctor to send the cause of death certificate to the Registrar, as medical institutions must now do. |
| Your application was rejected by the Registrar | File an appeal to the District Registrar within 30 days; the appeal must be decided within 90 days. |
